Multiple program areas offer site or application-level opportunities, focused on capacity, quality, and sustainability for rural providers. Event schedule: - 303 E. 17th Ave. Suite 1100 Denver, CO 80203 RHTP Grant Program Areas and Sample Activities , 2026 The sample activities listed within each initiative are illustrative and are not intended to be exhaustive. Many projects may support multiple Rural Health Transformation Program goals and could reasonably align with more than one funding category. Applicants should select the initiative that best represents the primary purpose and intended outcomes of the proposed project. Projects may incorporate activities that overlap with other programs when those activities are necessary to achieve the goals of the selected program. For example, workforce training may support telehealth implementation, telehealth investments may support chronic disease management, and care coordination activities may support both whole person health and value-based payment readiness. Funding Category: The Department reserves the right to determine the most appropriate funding category for proposed activities and may recommend alignment with a different initiative when appropriate. Funding Amount: The Department anticipates making Budget Year One awards generally ranging from $1 million to $3 million. Applicants are encouraged to request funding amounts that are reasonable and proportional to the proposed scope of work. Requests outside this range may be considered when supported by a clear justification and available funding. RFA Total: $160,295,755 Whole Person Health and Support for Alternative Care Models Amount - $40,011,564 The Department anticipates awarding up to $500,000 per site to a maximum of five sites identified within a single application. Goal - Support rural providers and consortiums in designing, implementing, and sustaining whole person health and alternative care models that improve access, quality, care coordination, and long- term financial stability. Improving health care equity, access and outcomes for the people we serve while saving Coloradans money on health care and driving value for Colorado. hcpf.colorado.gov - June 15 - RHTP Grant Program RFA Sample Activities Page 2 of 18 Description - This activity will fund rural providers and consortiums to conduct analysis and planning for financial stability, quality improvement, alternate care, or other planning activities related to developing and implementing whole person health. Rural providers will gain an understanding into the current capabilities and needs to further develop and sustain programs. Rural providers will train staff on one or more new models. Rural providers may purchase the infrastructure necessary to implement or sustain their whole person health or alternate care activities. This may include technology investments, costs for minor renovations, establishment of remote nutritional and health services, alternative care delivery, outpatient and ambulatory withdrawal management services, costs of becoming an Federally Qualified Health Center (FQHC), Comprehensive Safety Net Provider (CSNP), or other cost based provider, and sustainability. Any equipment costs must be related to creating new models of care and access points. Funding will not include new electronic health record systems (EHRs). Sample activities may include but are not limited to: Assessment, Planning, and Practice Transformation ● Conduct financial and operational assessments to identify gaps in care delivery ● Conduct community health needs assessments and service gap analyses to inform whole person health strategies and implement recommendations ● Develop plans for cost-based provider transitions such as becoming a Federally Qualified Health Center (FQHC), Certified Community Behavioral Health Clinic (CCBHC), or other sustainable provider model ● Train staff on alternative care models, including integrated care, home visiting programs, telehealth specialty care, and whole person health approaches Care Coordination and Integrated Care ● Implement care coordination programs for individuals with complex medical, behavioral health, or social needs ● Implement trauma-informed care and culturally responsive care models across clinical and community settings ● Expand community health worker, peer support specialist, care navigator, or other community-based workforce programs ● Implement team-based care models such as Prenatal Plus and other integrated care approaches Population Health and Prevention Improving health care equity, access and outcomes for the people we serve while saving Coloradans money on health care and driving value for Colorado. hcpf.colorado.gov

I fully support and commit to this Rural Health Transformation plan to fuel a creative and impactful transformation of health care for rural residents across our state. The State of Colorado is dedicated to pursuing a collaborative approach with key stakeholders to achieve the following strategic goals: Make rural America healthy again; reducing chronic disease; sustainable access; workforce development; innovative care; and tech innovation. State Commitment and Lead Agency The State of Colorado recognizes the urgent need to strengthen and transform health care delivery and access for our rural communities. Through the Rural Health Transformation Plan, we will transform rural health care systems by improving the sustainability of providers, addressing health equity, and advancing innovative care delivery models. I hereby designate the Colorado Department of Health Care Policy & Financing (HCPF) as the lead agency responsible for submitting the application and implementing the State of Colorado’s plan. HCPF will leverage its experience in serving more than 21% of the state’s population, our partnerships with rural providers, and collaborative efforts with engaged Colorado stakeholders. This agency will coordinate all aspects of planning, stakeholder engagement, and program oversight in collaboration with key state and community partners. HCPF oversees the administration of Health First Colorado (Colorado’s Medicaid program) and numerous value-based programs that span the State of Colorado. The financial support available through this grant will enable Colorado to elevate CMS’s strategic goals and pursue transformative outcomes for our rural communities. Rural Health Transformation Program funding will be requested for federally-defined rural and frontier counties in Colorado that continue to identify significant needs and gaps in accessible health care. Our interest in funding is a direct response to rural community needs, as demonstrated by data - November 4 - supplied by the Colorado Rural Health Center and surveys from numerous providers and partners across the state. Collaboration and Stakeholder Engagement From the outset, my administration has made it a priority to ensure that the Colorado Rural Health Transformation is driven by the voices of rural Coloradans. The State will integrate stakeholder input directly into decision-making through a transparent and inclusive governance structure designed to reflect the diversity of our rural health ecosystem. We have engaged key stakeholders through our planning process via listening sessions, ideation and feedback sessions, webinars, surveys, and interviews.
