The program targets gaps in rural oral health care through investments in technology and workflow improvements. Eligible projects focus on implementing new dental technologies, equipment, or software that enhance workflow, care, or accessibility for rural populations. Up to 20% of requested funds may be used for minor renovations (with prior approval), and no more than 8% may be allocated for administrative expenses. Applicant wages/salaries are not fundable. Projects must serve rural or semi-rural Wisconsin counties, excluding Milwaukee County. Applicants can apply to multiple Rural Health Transformation Program (RHTP) opportunities.
Eligible applicants include rural dental clinics, Federally Qualified Health Centers (FQHCs), Critical Access Hospitals, rural hospitals, nonprofit dental clinics, tribal dental clinics, and organizations providing dental services located in Wisconsin outside of Milwaukee County. Projects must serve rural or semi-rural counties as defined in Exhibit 1 of the RFA.
