The program requires screening of at least 165 residents, enrollment of a minimum of 50 into a 12-month CDC-recognized lifestyle intervention, and at least 60% participation of minoritized groups. Grantees must provide outreach, partnership development, program delivery, and meet all CDC DPRP and state/federal requirements. Total first-year funding: $195,000 (100% federal through CMS Rural Health Transformation Grant). Applications are to be submitted via email to OMHHD@dhhs.nc.gov by 5:00 PM ET July 7, 2026.
Eligible applicants include public or private nonprofit/for-profit organizations with 501(c)(3) status, local health departments, Tribal health programs, community- or faith-based organizations, academic institutions, or government entities recognized under North Carolina law.
