The North Carolina Department of Health and Human Services (NCDHHS), Office of Minority Health and Health Disparities, seeks qualified applicants to deliver the Minority Diabetes Prevention Program in Region 8 (Bladen, Brunswick, Columbus, Duplin, New Hanover, Onslow, Pender, Robeson, Sampson counties). Grantees must screen at least 165 residents for prediabetes, enroll a minimum of 50 participants in a CDC-recognized 12-month lifestyle change program, ensure at least 60% participation from minoritized populations, and coordinate at least 4 12-month lifestyle class series. The project must maintain CDC DPRP recognition, establish collaborative partnerships (including Tribal engagement when applicable), provide outreach and quality improvement, submit a Project Plan per Division guidance, and comply with fiscal/programmatic reporting requirements. Year 1 funding is $195,105 (100% federal via CMS RHT Program). Per Addendum 1 (issued June 19, 2026), the submission deadline was extended to July 10, 2026, 5:00 PM ET, and participant eligibility was clarified to require CDC DPRP eligibility (qualifying prediabetes blood test, history of gestational diabetes, or a positive prediabetes risk screening) in addition to BMI/age/no-prior-diagnosis criteria. Per Addendum 2 (issued July 2, 2026), Year 1 award amount is revised to $195,105 and the application deadline is extended to July 17, 2026, 5:00 PM ET. Applications are due via email to OMHHD@dhhs.nc.gov.
Applicants must be public or private nonprofits, for-profit organizations with 501(c)(3) status, local health departments, Tribal health programs, community- or faith-based organizations, academic institutions, or other government entities recognized under North Carolina law. Applicants must demonstrate experience implementing community-based health or chronic disease prevention programs and administrative capacity for a 12-month CDC DPRP-compliant lifestyle intervention. Individuals and foreign organizations are not eligible.
