Align participant eligibility standards with CDC Diabetes Prevention Recognition Program
This document introduces the North Carolina ROOTS Hub Leads, organizations selected to coordinate and implement NCRHTP initiatives across six rural Medicaid regions. Each Hub will form local partnerships to tailor strategies in areas such as care coordination, digital health, primary care access, and workforce development. The initiative spans a five-year period and will focus on strengthening rural healthcare infrastructure tailored to regional needs.
