NC DHHS Office of Rural Health sought six regional lead organizations (one per NC Medicaid region) to serve as NC ROOTS Hub Leads, responsible for managing and coordinating rural health transformation initiatives—including primary care, behavioral health, workforce development, value-based payment, and digital health—for roughly 3 million rural North Carolinians. Funding for GAP 1 totals $7,513,062 (approximately $1,252,177 per hub for 17 months). Awards were announced May 1, 2026. Five organizations were selected: Impact Health (Region 1), Trillium Health Resources (Regions 2 & 5), Vaya Health (Region 3), UNC Hospitals (Region 4), and Access East Inc. (Region 6). Program period: June 1, 2026 – October 30, 2027.
Eligibility: Public or private entities with a Unique Entity Identifier (UEI), at least 3 years’ presence in North Carolina, and fiduciary capacity.
