Eligibility

Eligible applicants include Missouri's state agencies, departments, or designated statewide rural health organizations authorized by the state government to implement rural healthcare transformation. All applicants must demonstrate capacity for statewide impact and compliance with CMS cooperative agreement requirements.

Requirements Checklist

• Applicants must submit: (1) a comprehensive rural health transformation plan aligned with CMS strategic goals; (2) evidence of statewide stakeholder engagement; (3) detailed implementation timeline and budget; (4) outcome metrics for workforce, technology, chronic disease, maternal health, and behavioral health; (5) assurances of compliance with federal cooperative agreement terms. Additional attachments may be specified in the NOFO.
