Synopsis As part of RHTP’s Transforming Rural Health through Partnerships Initiative, the Chronic Disease Collaborative Care Fund aims to reimagine Michigan's rural health landscape by testing and scaling new, community-driven approaches to care. This funding opportunity will support Michigan’s rural health providers and community organizations in advancing innovative, collaborative approaches to chronic disease prevention and management. This funding opportunity is designed to pilot and scale evidence based interventions that improve outcomes for diabetes, hypertension, and other chronic conditions, while leveraging existing rural health and state networks and strengthening cross sector partnerships. Integration of community health workers and behavioral health supports are included as complementary strategies to enhance care delivery.
