Michigan's Rural Health Transformation Program remains in the implementing phase. In July 2026, MDHHS issued multiple new grant funding opportunities for the Workforce for Wellness Initiative, Chronic Disease Collaborative Care Fund, and Improving Care Delivery and Access through Transportation. Multiple RHTP-funded programs are currently accepting applications, and local organizations continue to execute projects in alignment with workforce, prevention, and care access priorities. State opportunities: 31 active (17 open, 14 closing soon, 3 past/closed), $85.2M listed funding. Key opportunities: Michigan Workforce for Wellness: Rural Health Provider, Recruitment, and Retention Program; Michigan Workforce for Wellness: Bachelor of Social Work to Clinical Master of Social Work Stipend Program; Michigan Workforce for Wellness: University-Led Master of Social Work Award Program. Strategy alignment: supports workforce, prevention, care access priorities.
Michigan’s RHTP pursues system-level transformation through targeted federal and state investments, administered by MDHHS, to address unique rural health needs. The strategy centers on grant-funded initiatives that build collaborative regional networks, expand and stabilize the rural health workforce, improve chronic disease care, modernize health IT infrastructure, and implement community-based service models such as enhanced transportation options. Oversight is maintained via an advisory council and transparent subrecipient reporting, with phased implementation and ongoing performance monitoring.
Model
A grant-driven, hub-and-spoke care delivery framework leveraging regional partnerships, robust rural workforce pipelines, interoperable technology, innovative care models, and dedicated tribal health funding, with all implementation steered by MDHHS and its Office of Rural Health Transformation.
Key Initiatives
Timeline: Funding and phased implementation from FY2026 through FY2030; state planning and application in 2025; CMS approval in 2026; grant competitions and advisory council convening in 2026; ongoing evaluation and project implementation through 2030.
Transportation for rural health access: Barriers to reliable transportation limit rural residents’ access to medical appointments and ongoing care. The program aims to fund solutions that address transportation gaps.
Chronic disease collaborative care: Rural communities face higher rates of chronic disease and limited access to coordinated care. Funding is available to support collaborative approaches in managing chronic conditions.
Workforce for wellness: There are persistent workforce shortages in rural Michigan affecting health outcomes. The initiative supports recruitment and retention of rural health professionals.
