Synopsis As part of RHTP’s Transforming Rural Health through Partnerships Initiative, the Collaborative Care Integration and Sustainability Funds aims to reimagine Michigan's rural health landscape by testing and scaling new, community-driven approaches to care. This funding opportunity will support innovative rural health efforts that expand access to care and improve population health outcomes across the lifespan.
