Healthy Horizons RFA (New Mexico RHT Program)

Summary

Funding opportunity under the NM Rural Health Transformation Program (Healthy Horizons), supporting expansion of specialty, maternal, behavioral health, and chronic disease services in rural, frontier, and tribal New Mexico communities. Projects must demonstrate sustainability and include multi-sector partnerships.

Eligibility

Applicants must be New Mexico-based public, nonprofit, or tribal organizations operating licensed healthcare facilities, clinics, or provider groups within rural, frontier, or tribal service areas. For-profit entities and organizations outside New Mexico are not eligible.

Requirements Checklist

• Applicants must submit: a signed cover letter; a completed Application Form (Appendix A); a Project Narrative (max 10 pages); a detailed Budget Spreadsheet (Appendix B); a Financial Sustainability Plan; Letters of Commitment from all partners; and Organizational Certifications. All required forms are supplied in the RFA appendices.
