Requirements Checklist

• Applicants must propose new or expanded collaborative initiatives that improve care coordination and measurable health outcomes in rural or semi-rural Wisconsin. Proposals must include evidence of community need, a clear logic linking project activities to outcomes, a sustainability plan, and detail on the partnership structure. Applicants must submit all required forms, letters of support from formal partners (as applicable), and a planning period budget using the Coordinating Care Across Wisconsin Grant Budget Template. Funded activities must comply with DHS Allowable Cost Policy and applicable federal requirements; limitations apply to clinical services, capital, and administrative costs.
