Colorado RHTP: Whole Person Health and Alternative Care Models

Summary

Colorado RHTP activity supporting planning training and infrastructure for whole person health and alternative care models in rural and frontier communities.

Eligibility

Eligible lead applicants include qualified rural health care providers such as CAHs, REHs, rural PPS and sole community hospitals, FQHC or look-alike, RHC, CMHC or CCBHC, OTP, physician or NPP groups, and EMS organizations. Collaborative and third-party applications require required attestations.

Requirements Checklist

• Applicants must demonstrate plans for implementing, training, and building infrastructure for whole-person health and alternative care models to improve access and sustainability. Collaboration with advisory boards and outreach efforts are expected.
