Colorado RHTP: EMS Transport Coordination and Coverage

Summary

Colorado RHTP activity to improve rural EMS coordination coverage and integration across providers.

Eligibility

Eligible lead applicants include qualified rural health care providers such as CAHs, REHs, rural PPS and sole community hospitals, FQHC or look-alike, RHC, CMHC or CCBHC, OTP, physician or NPP groups, and EMS organizations. Collaborative and third-party applications require required attestations.

Requirements Checklist

• Applicants must detail the proposed use of funds to expand and improve rural EMS coordination, update technology, and integrate services across providers. Evidence of collaborative partnerships among relevant local healthcare stakeholders and alignment with the overall RHTP strategy is required. Activities must support better coordination of EMS transport and coverage for rural populations.
