Colorado RHTP: Advisory Board and Outreach for Hospitals

Summary

Colorado RHTP activity to coordinate statewide advisory board and outreach collaboration among rural providers behavioral health tribal organizations and state partners.

Eligibility

Eligible lead applicants include qualified rural health care providers such as CAHs, REHs, rural PPS and sole community hospitals, FQHC or look-alike, RHC, CMHC or CCBHC, OTP, physician or NPP groups, and EMS organizations. Collaborative and third-party applications require required attestations.

Requirements Checklist

• Recipients will coordinate a statewide advisory board and conduct outreach activities to gather input, share information, and support collaboration across eligible rural health organizations, as stipulated in the RHTP Grant Program RFA Activities Overview.
