The report analyzes the relationship between commercial reimbursement rates and the financial health of rural hospitals in Kansas, Oklahoma, and Nebraska. It finds that inadequate commercial rates, especially from dominant payers like Blue Cross Blue Shield of Kansas, are a primary driver of rural hospital distress. Modeling shows that higher commercial reimbursements could shift hospitals from loss to positive margins, supporting essential rural healthcare services.

This report documents worsening access to maternity care in rural Kansas, with 46% of women living in counties without services and 59% of the state's counties lacking local inpatient maternity care. Western Kansas faces the highest proportional need, often requiring women to travel long distances for care. Coverage improvements, such as Medicaid reimbursing doulas and lactation support, are helping but are not enough to address the statewide workforce, facility, and provider gaps threatening maternal and infant outcomes.
