Requirements Checklist

• Submit revised budget and narrative for period 1 budget category allocations by January 30, 2026.
• Submit required quarterly and annual progress reports through GrantSolutions.
• Comply with all Terms and Conditions in the NOFO and award letter.
• Participate in annual and quarterly program reviews and technical meetings with CMS.
• Submit Non-Competing Continuation (NCC) applications each budget period for continued funding.
