25RFP-26-0903002-0002 To Provide Additional Long-Term Care Nursing Facility Capacity in SD. South Dakota Department of Health is seeking proposals from licensed long-term care providers for reallocating up to 991 nursing facility beds statewide, pursuant to SDCL §§ 34-12-35.6–35.8. Proposals are evaluated as received and must address special care needs. Providers are also encouraged to expand home and community-based services. Contracts may be awarded for one year with up to four one-year renewals, subject to funding availability and performance benchmarks.
Licensed long-term care providers located in South Dakota are eligible. Preference is given to proposals addressing identified special care needs, and to applicants proposing expansion of home and community-based services. Compliance with SDCL §§ 34-12-35.6–35.8 is required. Applicants expending $1,000,000 or more in federal awards in a fiscal year must comply with single audit requirements per 2 CFR 200 Subpart F and the state subrecipient agreement Section A.7.
