The Louisiana Department of Health (LDH), through the Office of Rural Health Transformation and Sustainability, is issuing this Notice of Funding Opportunity (NOFO) under the Rural Health Transformation Program (RHTP) to support targeted investments in telehealth infrastructure for rural Louisiana. The program funds upgrades and implementation of telehealth networks, remote patient monitoring (RPM), and connected care platforms to expand access to specialty, behavioral health, prenatal, and chronic care services in rural and underserved parishes. Projects must demonstrate measurable improvements in rural health outcomes.
Applicants must meet rural-serving status under at least one of the following pathways: 1) Organization located in a federally designated rural area (HRSA FORHP criteria), including non-metropolitan counties and RUCA-aligned rural census tracts; 2) CMS-designated rural hospital status; or 3) Providers serving a majority rural population based on RUCA methodology, even if not physically located in a rural area.
