Requirements Checklist

• Applicants must:
• Register with the SD Office of Procurement Management and complete the SB198 Vendor Certification Form
• Ensure active SAM.gov registration and Unique Entity Identifier (UEI)
• Align projects with grant period dates and ensure completion by the spending deadline
• Submit a needs assessment, project narrative for each funding tier requested, implementation plan, and sustainability plan
• Use sample worksheet for required applicant profile, project leads, and budget
• Submit project proposals with technology plans and budget justification.
