The South Carolina Department of Health and Human Services is soliciting applications for the Shoring Up to Sustainability Initiative under the RHTP. This RFA supports RHT efforts focused on facility enhancements, masterclass workforce training, and healthcare workforce recruitment/retention. Eligible applicants are entities serving rural South Carolina, including providers and organizations aiming to improve operational and financial resilience in rural health systems. Invests in rural healthcare system sustainability, workforce recruitment and retention (including incentive payments for physicians and advanced practitioners), facility upgrades and provider training. May include renovations and improvements to spaces where services are provided.
Eligibility
Rural health care organizations and providers
Healthcare providers, community-based organizations, non-profit organizations, local government and public health entities, and other entities committed to improving rural health care delivery.
Eligible applicants must be one of the following: Healthcare Providers serving rural South Carolina communities (including hospitals, clinics, federally qualified health centers, EMS providers, and pharmacies); South Carolina based Community-Based Organizations (CBOs) with a demonstrated role in health access or digital health support; South Carolina based Local Government Entities or Public Health Agencies supporting rural health infrastructure; South Carolina based Nonprofit Organizations with a mission aligned to improving rural health outcomes; South Carolina based Educational Institutions (universities, technical colleges, and high schools) engaged in healthcare workforce development; Professional Associations and Licensing Boards supporting healthcare training and certification; Consortia or Collaboratives formed with South Carolina based partners to strengthen rural healthcare infrastructure and workforce capacity. AND Must serve rural communities in South Carolina, as defined by HRSA.

Renovation projects must be completed by Sept. 30, 2027.
Submissions detailing workforce incentives must identify number of individuals supported.
