School-Based Health Center Expansion Initiative – Kent and Sussex Counties

Summary

Competitive funding to establish and operate at least four school-based health centers (SBHCs) in rural elementary and middle schools in Kent and Sussex Counties, Delaware. This expansion initiative aims to improve health outcomes, educational success, and reduce disparities by providing comprehensive, integrated physical and behavioral health care directly to students and families. Required services include enhanced behavioral health, preventive screenings, health education, and coordination with community providers. The goal is to serve at least 400 patients by year three. Medicaid billing required for sustainability.

Eligibility

Eligible applicants include qualified healthcare organizations, providers, and entities with capacity to establish and operate SBHCs meeting the scope of work outlined in the RFP. Vendors must demonstrate experience in school-based and community-based health care delivery.

Requirements Checklist
