Eligibility

Eligible applicants include State, Local, and Indian Tribal Governments; institutions of higher education; non-profit organizations (including faith-based, community-based, and tribal organizations); hospitals; and other healthcare organizations located in Ohio with at least 5 years of experience delivering health services in one or more of the 73 counties of focus. Projects must benefit residents of those 73 rural counties as described in Appendix A of the RFP.

Requirements Checklist

• Applicants must submit a technical proposal and a completed cost proposal form. Proposals must describe the establishment or expansion of a clinically integrated network (CIN) or rural health hub, detail plans for governance, partnerships, data sharing, workforce development, and sustainability, and delineate budget allocations specifically increasing access to maternal care in rural communities, if applicable. Mandatory documentation includes proof of organizational leadership support, evidence of required experience, and clinician participation. Proposals must be submitted by email to Procurement@odh.ohio.gov per detailed RFP instructions.
