Requirements Checklist

• Submit a narrative proposal including: detailed staffing plan, 5-year implementation plan and timeline, stakeholder engagement plan, metrics and evaluation plan, sustainability plan, itemized budget and budget narrative.
• Deliver annual Rural Healthcare Academies, each enrolling at least 25 rural high school students per location in each RHTP region.
• Establish partnerships with required stakeholders in each region.
• Submit monthly, quarterly, and annual reports outlining outcomes and metrics.
• Submit a narrative proposal including: detailed staffing plan, 5-year implementation plan and timeline, stakeholder engagement plan, metrics and evaluation plan, sustainability plan, itemized budget and budget narrative.
• Deliver annual 8-week Rural Healthcare Academies enrolling at least 25 rural high school students per location in all eight regions.
• Provide monthly, quarterly, and annual reports tracking key outcomes.
• Engage clinical healthcare employers, behavioral and oral health employers, high schools, workforce councils, and community-based organizations.
• Plan, implement, and evaluate 8-week summer Rural Healthcare Academies in all 8 RHTP regions.
• Enroll at least 25 rural high school students per location annually.
• Provide classroom, hands-on, and employer-engaged learning.
• Submit all required plans and reports as detailed in the RFA.
