Rural Health Transformation Program - Telehealth

Summary

This subrecipient grant opportunity supports Vermont medical practices and provider consortiums in acquiring, implementing, and using telehealth technology solutions that enable secure, high-quality virtual care delivery. The initiative aims to improve access, continuity, and resilience of Vermont’s rural health system, especially for rural and underserved communities. Funding is available to support secure telehealth platforms, connected diagnostic tools, infrastructure, technical assistance, and training on technology selection, implementation, cybersecurity, and compliance. The grant is part of a five-year initiative begun in 2026, with $195 million available in Year One.

Eligibility

Eligible applicants are rural and rural-serving Vermont healthcare providers and provider consortiums. Applicants do not need to be licensed or reside in Vermont but must meet all eligibility requirements as defined in NOFO section 2.1.

Requirements Checklist

• Applicants must submit proposals that address secure, interoperable telehealth platform integration (excluding broadband), and track outcome metrics including practices acquiring telehealth technology and unique patients served. Applications will be evaluated on service to rural ZIP codes, quality of implementation plan, prior experience, sustainability approach, and evaluation strategy (scoring rubric in NOFO). Separate applications are required for telehealth and eConsult grants.
