the Rural Health Transformation Program (RHTP). Activities funded include technical assistance, training, planning, infrastructure development, telehealth, equipment, and remodeling to fill service delivery gaps and improve outcomes for Medicaid members and rural/tribal residents. About $15 million is available, with approximately 50 awards. All projects must serve primarily rural or tribal ND citizens and demonstrate measurable, sustainable improvements in health care access and outcomes.
Eligible applicants are rural/tribal communities and related organizations (see main eligibility text); urban entities must serve at least 50% rural population. Eligible entities include hospitals, clinics, home care providers, long-term care facilities, behavioral health providers, tribes and tribal health organizations, EMS providers, other health care providers, transportation providers, local public health, nonprofit organizations, education systems, and community organizations. For-profit providers are eligible for subrecipient work but not direct awards.
