Requirements Checklist

• Organizations must register as suppliers in OregonBuys prior to application. Proposals are submitted via a dedicated Smartsheet link (provided post-registration), using required forms: Document A (Proposer Information and Certification), Document B (Project Plan Narrative and Timeline, one per project), Document C (Grant Budget Narrative, one per project), and Document F (Disclosure Exemption Statement). Each lead applicant may submit one proposal, with up to three distinct projects. Projects must address at least one of the five RHTP initiative areas, serve rural Oregon, include outcome measures, targets, and project timelines, and meet the specified minimum and maximum funding thresholds. Participation in program data collection, reporting, technical assistance, and regional planning activities is required. All activities and costs must comply with the RFGP guidelines.

Submission Instructions

Register in OregonBuys as a supplier. After registration, access the Smartsheet link provided by OHA to submit required proposal documents and attachments. All required forms must be uploaded via Smartsheet; proposals submitted through other means will not be considered.
