Rural Health Transformation Program (RHTP)

Summary

The Rural Health Transformation Program (RHTP) provides $50 billion in federal funding to support state-level system transformation, rural hospital and Medicaid innovation, workforce strategies, quality improvement, and the implementation of health IT and new care models across all U.S. States (FY2026–2030). States will receive a mix of baseline and competitive funding distributed over five budget periods.

Eligibility

Eligible applicants are State governments from all 50 U.S. States. Local governments, territories, and tribal entities are not eligible to apply.

Requirements Checklist

• Applicants must complete registration with SAM.gov and Grants.gov prior to applying. Applications must include all forms and documentation as outlined in the 'Application Contents and Format' section of the NOFO. States must demonstrate capacity to implement rural health transformation strategies as described in the program's objectives.
