The Rural Health Transformation Program (RHTP) dedicates $50 billion in federal funding to transform rural healthcare delivery nationwide from FY2026 to FY2030. The program supports state-based innovation, sustainable care access, workforce development, care coordination, and technology upgrades. Funds are distributed to states via cooperative agreements and focus on improving healthcare access, quality, and outcomes through evidence-based interventions, sustainable rural provider models, and technology-driven solutions.
Eligible applicants are State governments (all 50 U.S. States and the District of Columbia). Only one application per state will be accepted. States must propose statewide rural health transformation plans in accordance with Section 71401 of Public Law 119-21 (One Big Beautiful Bill Act).
