Rural Health Transformation Program (RHTP)

Summary

The Rural Health Transformation Program (RHTP) dedicates $50 billion in federal funding to transform rural healthcare delivery nationwide from FY2026 to FY2030. The program supports state-based innovation, sustainable care access, workforce development, care coordination, and technology upgrades. Funds are distributed to states via cooperative agreements and focus on improving healthcare access, quality, and outcomes through evidence-based interventions, sustainable rural provider models, and technology-driven solutions.

Eligibility

Eligible applicants are State governments (all 50 U.S. States and the District of Columbia). Only one application per state will be accepted. States must propose statewide rural health transformation plans in accordance with Section 71401 of Public Law 119-21 (One Big Beautiful Bill Act).

Requirements Checklist

• Applicants must submit a comprehensive rural health transformation plan detailing statewide strategies for addressing healthcare access, quality improvement, provider sustainability, technology upgrades, workforce development, care coordination, and population health. Applications must include project abstracts, proposed outcomes, budget justifications, required certifications, and a commitment to comply with federal reporting and oversight requirements.
