Eligibility

Eligible applicants are healthcare provider organizations and provider consortiums serving rural Vermont communities. Applications must demonstrate ability to implement RPM technologies, meet interoperability and technical standards, and serve rural and underserved populations as defined in the NOFO.

Requirements Checklist

• Applicants must submit a complete application by the deadline, demonstrate project feasibility, sustainability, and alignment with program goals for rural transformation. Proposals shall detail the selected RPM technology, implementation plan, provider and patient recruitment, data reporting, and integration with electronic health records (EHR). No cost sharing or match is required. Refer to the NOFO for full requirements.
