Examples include Bluetooth-enabled, connected scales, blood pressure cuffs, pulse oximeters, glucose monitors, and cardiology RPM devices.
Eligible applicants are healthcare provider organizations and provider consortiums serving rural Vermont communities. Applications must demonstrate ability to implement RPM technologies, meet interoperability and technical standards, and serve rural and underserved populations as defined in the NOFO.
