Rural Health Transformation Program – Program Evaluator

Summary

the Rural Health Transformation Program (RHTP). The evaluator will perform annual and final reports to assess program outcomes and impact of activities funded by a $206,927,896.80 CMS/HHS award over a five-year period. Scope and deliverables are updated in Addendum #1, including reporting modifications. MWBE and IVOSB subcontractor participation is required in accordance with Indiana state goals and must be documented on the State of Indiana MWBE/IVOSB registry. All respondent eligibility must be met per state registration requirements before award.

Eligibility

Respondents must: (1) register a bidder profile with the State of Indiana via the Indiana Supplier Portal; (2) be registered with the Indiana Secretary of State prior to award; (3) submit MWBE and IVOSB subcontractor commitment forms as part of the proposal if participation is proposed (see Attachments A and A1); (4) ensure subcontractors are certified on the State of Indiana MWBE/IVOSB registry. Respondents themselves or any proposed subcontractors must meet certification listed at https://www.in.gov/idoa/mwbe Participation through MWBE and IVOSB subcontractors is required to meet state diversity goals but not mandatory for evaluation scoring.

Requirements Checklist

• Proposal submissions must include required attachments: Attachment A (MWBE Subcontractor Commitment Form), Attachment A1 (IVOSB Subcontractor Commitment Form), Attachment D (Cost Proposal), Attachment F (Respondent Information & Attestation Form), Attachment G (Question & Inquiry Process Form), and optionally Attachment I (Pre-Proposal Network Opportunities Form). Scope of work and deliverables have been updated by Addendum #1 and must be addressed. MWBE and IVOSB participation must be documented according to forms, with amounts matched to Attachment D. All questions must use Attachment G and be submitted to rfp@idoa.in.gov by deadlines specified. All vendor and subcontractor certifications and compliance with Indiana statutes must be demonstrated at submission.
