Eligibility

Eligible applicants include primary care and specialist healthcare providers serving Vermont, as well as provider consortia representing these groups. To qualify, applicants must serve patient populations where at least 20% are rural residents, measured on a site-by-site basis. Non-provider entities acting as consortia on behalf of healthcare providers are also eligible if they represent or provide shared services to eligible members. Applications must be led by a single entity holding a Unique Entity Identifier (UEI).

Requirements Checklist

Required documentation includes a project narrative, budget, budget justification, evidence of rural population served, and identification of partner providers if applying as a consortium.
