This subrecipient grant will support statewide implementation, management, and operation of a digital closed-loop referral platform to enhance care coordination among Vermont health care, mental health, substance use disorder, and social service providers. The system must enable real-time data sharing, referral outcome tracking, improved access to community social supports (including housing, food, and transportation), and increased cost-efficiency. The platform seeks to reduce service duplication, strengthen provider accountability, and deliver person-centered care for rural Vermonters.
Eligible applicants are nonprofit organizations, for-profit entities, government entities, or consortia with proven capacity and experience in managing large-scale digital health and social service technology solutions within Vermont or comparable environments. Applicants must demonstrate ability to serve as program manager for procurement, implementation, ongoing management, maintenance, and stakeholder engagement for a statewide closed-loop referral system, as specified in the Notice of Funding Opportunity (ClosedLoopReferral_NOFO_0007.pdf, Section 3.1).
