Rural Health Transformation Program - Adoption of a Statewide Closed Loop Referral System

Summary

This subrecipient grant supports a statewide closed-loop referral system for Vermont, strengthening coordination among health care, mental health, substance use disorder, and social service providers. The platform must facilitate real-time data sharing, referral outcome tracking, improved access to community social supports (housing, food, transportation), and increased cost-efficiency. It aims to reduce duplication of services, improve provider accountability, and advance seamless, person-centered care for rural Vermonters. Solution must comply with HIPAA, 42 CFR Part 2, and federal interoperability standards. Implementation includes stakeholder engagement, onboarding, training, quarterly reporting, operational readiness, and a sustainability plan.

Eligibility

Eligible applicants are nonprofit organizations, for-profit entities, government entities, or consortia with a proven track record in managing statewide digital health and social service technology solutions for Vermont or similar settings. Applicants must have full SAM registration and a valid Unique Entity Identifier (UEI) and must demonstrate readiness to serve as program manager for procurement, implementation, management, maintenance, and stakeholder engagement for statewide closed-loop referral systems, as specified in ClosedLoopReferral_NOFO_0007.pdf Section 2.1.
