The Georgia Rural Health Transformation Program (GREAT Health) Initiative 3: Connecting to Care to Improve Healthcare Access – Telehealth Enhancements supports projects that modernize digital infrastructure, expand specialty and behavioral health access, and build workforce capacity for rural healthcare providers in Georgia. Up to $10,000,000 is available in Year 1, with funds to expand telehealth delivery and sustainability. Priority will be given to rural hospitals previously participating in the AHEAD Model pre-implementation. All awarded funds must be expended by September 30, 2027; unallowable costs include new construction, services reimbursable by insurance, food/meals, pre-award costs, or duplicate funding sources. Applicants must demonstrate sustainability beyond the grant period.
Eligible applicants include rural hospitals, healthcare providers, and community organizations serving rural or partially rural counties defined in DCH’s application to CMS as eligible rural areas. Priority will be given to hospitals that previously submitted a signed Letter of Intent for the AHEAD Model pre-implementation to DCH. See FOA for complete list of eligible counties.
