The South Dakota Rural Health Transformation (RHT) Initiative is soliciting applications for the Rural BH Modernization (CCBHC) Infrastructure & Sustainability Grant (RFA #26-08RHT-016), with $9.7 million in federal funds available for Budget Period 1. This opportunity supports system-level behavioral health improvements for development, expansion, and sustainability of Certified Community Behavioral Health Clinics (CCBHC), explicitly strengthening access and quality of care in rural and frontier South Dakota. Funding priorities include provider capacity building, crisis services expansion, IT infrastructure development, and workforce development. Eligible applicants are exclusively organizations selected for the 2026 CCBHC Planning Grant Cohort. Applicants must demonstrate readiness, advance CCBHC certification progress, report measurable outcomes, and describe sustainability plans post-award. There is no cost share/match requirement. Applications are due July 15, 2026, via the South Dakota eSM Solutions Posting Board portal. The grant support period runs from agreement execution (~Aug 1, 2026) through July 30, 2027, with potential renewals up to five years.
