The South Carolina Department of Health and Human Services (SCDHHS) is launching the Rural Health Transformation Program: Wellness Within Reach Initiative, making $27.7M available via subawards to rural-serving organizations. The initiative focuses on deploying mobile health units and crisis response teams, and expanding community care sites to improve healthcare access, quality, and outcomes in rural areas. Eligible applicants must serve rural SC communities and demonstrate measurable impact for rural residents.
Eligible applicants must be one of the following: (i) Healthcare providers and systems (hospitals, rural health clinics, federally qualified health centers, primary care clinics); (ii) Public or volunteer EMS agencies; (iii) Pharmacies operating in rural SC; (iv) Community-based organizations based in SC providing health-related services in rural areas; (v) County/municipal public health agencies; (vi) Consortia/collaboratives formed with eligible SC-based partners. All applicants must serve rural communities in South Carolina (as defined by HRSA). Proposed activities must directly benefit rural residents and demonstrate measurable impact.
