Competitive grants for Minnesota rural organizations to establish Telehealth Access Points—dedicated spaces in community sites (schools, pharmacies, libraries, tribal clinics, etc.) where rural residents can access telehealth for primary care, behavioral health, and oral health. Funding supports technology, site upgrades, staff, and care navigation to reduce barriers to healthcare access.
Eligible applicants include rural community organizations, schools, local public health agencies, tribal clinics, libraries, and rural health care providers within Minnesota. Applicants must demonstrate ability to host TAPs in accessible, trusted locations and serve rural populations.
