Provides startup funding for new or expanded rural and satellite clinics delivering behavioral, dental, or medical care. Supports creation or expansion of rural clinic locations, including eligible urban organizations opening rural service sites, strengthening access to comprehensive health care in underserved Florida rural communities. Refer to RFA 036-25/26 and Addenda for full eligibility, Super Region/county lists, and program priorities.
Eligibility
Eligible applicants include for-profit and not-for-profit Florida health care providers, community organizations, and other qualified entities located in or actively serving patients in eligible rural Super Region counties. Safety net, charitable, and free clinics are eligible. Applicants must document organizational capacity and eligibility, comply with all RFA requirements, and be prepared to serve rural-designated areas as listed. See RFA and Addenda.

Requirements Checklist

Budget must comply with RFA caps: mobile unit lease maximum $12,000/month, maximum 36 month lease, no major construction, renovation max 20% of total budget, no land leases, no direct clinician salary for patient care.
Provide a post-grant mobile unit transition plan if requesting a mobile unit.
Comply with all application guidance, blackout communications per s.287.057(25) F.S., and program rules.
