Stormont Vail HealthCare proposes PrairieLINK, a regional clinically integrated network and shared services infrastructure to strengthen rural hospital sustainability across northeastern and central Kansas through performance benchmarking, care coordination and value-based care alignment.

Great Plains Health Alliance

Great Plains Health Alliance and Pioneer Health Network will deploy Avel eCare telemedicine services to 31 rural Kansas hospitals across all six Kansas Hospital Association regions, providing inpatient, emergency and psychiatric tele-coverage.

Recovery Unlimited

Recovery Unlimited KS will create a technology-enabled substance use disorder treatment model linking community supervision, primary care and specialized treatment through Skyscape telehealth platform, establishing medical detox centers in south-central and western Kansas.

Garden City Community College

Garden City Community College will lead the "Frontline Futures" partnership to expand clinical training, launch a diagnostic medical sonography program, develop an accelerated radiology technology pathway, and build a regional healthcare workforce pipeline across western and southwestern Kansas.

Boston Consulting Group will support Kansas RHTP implementation with rapid response and infrastructure development, focusing on staffing and process improvements at KDHE.

Hutchinson Regional Healthcare System

Renovation of an idle nursing home facility on campus into an adult acute behavioral health unit and integrating with a new KU School of Medicine psychiatry residency program.

Gove County Medical Center

Replacement of an unreliable mobile MRI with a permanent fixed MRI system, restoring reliable diagnostic imaging access for this 21-bed critical access hospital (CAH) in western Kansas.

Clara Barton Medical Center

Construction of a sterile processing department, operating room (OR) dehumidification, and renovation of the rural health clinic to activate behavioral health space.

Pratt Regional Medical Center

Conversion of two endoscopy rooms to dual-purpose OR endosuites, expanding the Family Birth Suite, and launching the first breast MRI cancer screening program.

Citizen's Foundation (Citizens Health)

In partnership with the University of Kansas Cancer Center, Citizen's Foundation will establish a virtual tele-oncology program for nine northwestern rural Kansas counties and fund facility renovation, major diagnostic equipment (MRI, mammography, DEXA, ultrasound), and a Transforming Cancer Care clinical team.

Special Olympics Kansas

Iroquois Center for Human Development

Iroquois Center for Human Development will lead the Kansas Frontier Behavioral Health Integration Collaborative that will build an organized healthcare arrangement integrating behavioral health into primary care, emergency medicine and outpatient settings across frontier southwest Kansas.

High Plains Mental Health Center

High Plains Mental Health Center will deploy mobile mental health clinic vans (Be There initiative) across northwestern Kansas counties, with school liaison services and community health worker outreach through LiveWell Northwest Kansas.

Greeley County Health Services

Greeley County Health Services will develop the Western Kansas Shared Services Group, a regional collaborative involving hospitals in four communities to expand specialty care, develop a shared services model, implement shared staffing and build technology infrastructure across western Kansas.

Kansas Council on Developmental Disabilities

Kansas Council on Developmental Disabilities will partner with Mental Health America of South Central Kansas and two community developmental disabilities organizations to pilot implementation of the START model for individuals with intellectual and developmental disabilities.

AdventHealth Ottawa

AdventHealth Ottawa will establish a hub-and-spoke maternal/obstetric and specialty care delivery system with purpose-built mobile clinic units (prenatal/women's health and DEXA/bone density) and RN/APP-facilitated telehealth specialty outreach.

Labette County Medical Center

Labette County Medical Center and Southeast Kansas Health cooperative will build the region's first collaborative health care governance structure, develop behavioral health crisis infrastructure, and create a behavioral health crisis stabilization unit.

UKHS Care Collaborative Association will provide project management services as a sub-recipient, facilitating implementation with rural providers statewide.

Five founding providers will form AstraHealth Kansas, a shared services organization providing benchmarked clinical data, analytics dashboards and legal/governance infrastructure to rural hospitals across Kansas.

Mercy Hospital Pittsburg

Mercy Hospital Pittsburg will implement SMART-enabled inpatient rooms with virtual specialty access (Mercy Virtual) across emergency, inpatient and step-down settings, expanding telepsychiatry, teleradiology and specialty consultation to reduce avoidable transfers.

Kansas Health Science University

Kansas Health Science University will deploy two complementary telehealth technologies: a Drone Slit Lamp System for remote ophthalmology exams and a second diagnostic platform, in partnership with Bascom Palmer Eye Institute and Kansas College of Osteopathic Medicine.

Sunflower Foundation will recruit and train 90 rural primary care practices for behavioral health integration and provide ongoing learning collaboratives and technical assistance.

InterHab (CIDDH Curriculum)

InterHab and IntellectAbility will deploy the Curriculum in Intellectual and Developmental Disabilities (I/DD) Healthcare — a CME/CEU-accredited eLearning program — statewide to train 300+ rural licensed clinicians in evidence-based recognition and management of chronic conditions in I/DD patients.

Grace Team will deploy a regional behavioral health and dementia-capable workforce initiative across 14 rural long-term care facilities, providing two-day on-site training to improve skills in managing dementia-related behaviors and psychiatric symptoms, training approximately 800 frontline staff.
