The Memory Care Assessment Network (MCAN) competitive grant is part of the Tennessee Rural Health Transformation Program (RHTP), aiming to build memory care assessment capacity across rural Tennessee. It seeks to support rural hospitals, FQHCs, non-profit and community-based organizations, and eligible healthcare partners to enhance care for older adults and address rural health challenges.
Eligible applicants include rural hospitals, Federally Qualified Health Centers (FQHCs), non-profit and community-based organizations, and other eligible healthcare partners. Applicants must be registered with the State of Tennessee.
