RHTP HRP: Service Line Expansion and Co-Location

Summary

The Rural Health Transformation Program (RHTP) is a competitive grant supporting rural hospital service line development and co-location of health services across Tennessee. The RHTP aims to reverse rural hospital decline, expand care access, modernize health technology, and improve health outcomes for mothers, children, and older adults.

Eligibility

Rural hospitals and health systems, Federally Qualified Health Centers, non-profit and community-based organizations, and other eligible healthcare partners. Applicants must be registered with the State of Tennessee.

• Applicants must submit a complete application via the TDH Partner Portal and provide all required supporting documents as outlined in the online portal. Participation in at least one webinar or info session is strongly encouraged for potential applicants.
