RHTP HRP: Health Tech Innovation

Summary

The Rural Health Transformation Program (RHTP) Competitive Grant will fund innovative projects that improve rural healthcare access, hospital sustainability, and care quality for mothers, children, and older adults in rural Tennessee communities.

Eligibility

Rural hospitals and health systems, Federally Qualified Health Centers, non-profit and community-based organizations, and other eligible healthcare partners. Applicants must be registered with the State of Tennessee.

Requirements Checklist

• Projects must demonstrate alignment with the RHTP plan goals: reversing rural hospital decline, extending care access, modernizing health technology, and improving health outcomes. Applicants must provide a clear project plan, measurable objectives, and budget justification. All projects must serve rural communities in Tennessee.
