• Applicant Question Deadline: 07/10/2026 ET
• Contract Execution
• Minimum viable platform launch (Milestone 1): within 60 days of contract execution
• Routine use demonstration (Milestone 2): 4 months after contract execution
• Interim scale and performance demonstration (Milestone 3): 8 months after contract execution
• Year 1 performance report & sustainability plan (Milestone 4): End of Year 1 (12 months after contract execution)
• Monthly program reports due: 15th of the full month following grantee award and each month thereafter

This is designed to directly align with the evaluation criteria. a) Section 1: Organizational Capacity, Experience, and Readiness: describe your ability to implement the project and your readiness to comply with the requirements of this subaward based on your organization’s experience and capabilities. Applicants must provide evidence of existing deployments, including existing live clients, current referral volumes, number of active provider organizations or EHR instances, number of active CBOs or community-serving organizations, examples of closed-loop completion reporting, and resumes of key program members.
