Summary

the Rural Health Transformation Program (RHTP). The program, funded by a $209 million federal award from CMS, aims to strengthen rural health care through education and outreach to assist Medicare and Medicaid beneficiaries in selecting integrated health plans. Eligibility

Only current Florida Department of Management Services State Term Contract (STC) holders are eligible to apply. Responses are solicited exclusively from this pool per Ch. 287, Florida Statutes.
