the Rural Health Transformation Program (RHTP) Chronic Disease Management Program. This competitive opportunity provides $15.2 million for evidence-based or evidence-informed interventions addressing cardiovascular disease, cancer, respiratory illnesses, diabetes, and stroke in rural Oklahoma (see Attachment A for eligible communities). Funding will support personnel, clinical equipment, minor renovations, vehicles, program operations, and program evaluation. Awards must demonstrate compliance with CMS, measurable outcomes, and targeting of underserved rural communities.
Eligible applicants are rural hospitals, federally qualified health centers (FQHCs), tribal health organizations, non-profit health clinics, and community-based programs providing direct services to populations in OSDH-defined rural Oklahoma communities as listed in Attachment A. Direct services must be provided in eligible rural communities. Centralized or remote services (e.g., telehealth, administration) may qualify if they directly support program participants in eligible rural areas.
