RHTP Center of Excellence (CoE) Facility Participation Incentives

Summary

Rural and tribal hospitals, FQHCs, and RHCs can opt into the CoE initiative, committing to implement CoE recommendations and meet measurable improvements in care quality, patient access, and financial performance for incentive payments. DPHHS seeks contractor proposals to create a Center of Excellence for rural health, tasked with providing county- and facility-level recommendations, analyzing rural health supply and demand, and guiding strategic planning for Montana’s rural providers.

Eligibility

Rural & tribal hospitals, FQHCs, and RHCs in Montana. Interested vendors and subcontractors; organizations experienced in rural health strategy and management.

Requirements Checklist

• Commit to implement CoE recommendations.
• Achieve specified performance metrics in quality, access, and financial outcomes.
• Submit proposals via Montana DOA procurement portal.
• Provide evidence of technical expertise and change management capability.
