The RHTP 2026, issued by the New Jersey Office of Primary Care and Rural Health, supports projects that expand preventive services, deploy technology and telehealth, strengthen workforce capacity, and incentivize recruitment and retention in rural New Jersey healthcare settings. A total of $27.5 million is available across four main activity areas, aiming to improve health outcomes in rural census tracts with an emphasis on the uninsured and underinsured. No cost sharing or match is required. Applications are due by January 20, 2026, and must be submitted through the NJDOH SAGE Grants management portal.
Eligible applicants include providers and organizations serving residents in federally designated rural census tracts in New Jersey as well as those qualifying under state or alternative rural definitions. Entities must demonstrate the capacity to serve uninsured and underinsured rural populations. Eligible applicant types include, but are not limited to, local health agencies, federally qualified health centers, hospitals, non-profit organizations, academic institutions, and State agencies. Projects must take place within the geographic boundaries specified by the program.
