RFI for Outpatient Remote Monitoring services

Summary

The West Virginia Rural Health Transformation Program (RHTP) Office issued this Request for Information (RFI) to gather input from vendors on scalable, technology-driven outpatient remote monitoring solutions for rural and high-risk populations. Information collected will guide design and future procurements for the Connected Care Grid initiative.
