Requirements Checklist

• Applicants must provide a project narrative, staffing plan, timeline, and a budget using the provided Attachment A template. Required activities include delivering preventive dental hygiene services for children and adults, serving long-term care settings with appropriate training, integrating teledentistry as a core service, and coordinating with local dental professionals. Applications must include a formal resolution or letter of support from the LHD governing board, documentation of engagement with service sites and the dental community, and show readiness to report monthly encounters, comply with federal and state regulations, and participate in required DPH training. Administrative and indirect costs may not exceed 10% of the total award. At least 50% of project costs must be expended by the end of Year 1. Construction or major facility retrofitting is not allowed.
• Applicants must submit a narrative, detailed budget (using Attachment A), evidence of accreditation (or plans to achieve accreditation), partnership letters (where applicable), and assurances on non-supplantation of funds. All applications must clearly describe measurable program impacts and sustainability plans.
• Submit a one-page cover sheet with program and contact information
• Submit an application narrative (max 25 pages) addressing program intent, methodology, partnerships, barriers, evaluation and sustainability plans, and implementation timeline
• Include supporting documentation: letters of support, rural community engagement documentation, and required budget template
• Projects must adhere to CODA accreditation standards and expend at least 50% of start-up costs by September 30, 2027
• Budgets must comply with CMS guidance; indirect and total administrative costs capped at 10%

Applications will be considered on a rolling basis, but must be received by April 24, 2026 to be considered for FY27 funding. Applications must be submitted via email to pam.stein@ky.gov by 11:59 PM ET on May 29, 2026. Use the subject line: 'RHTP Dental Hygiene Application - [Institution Name]'.
