Requirements Checklist

• Submit required coversheet
• Budget narrative explaining requested costs
• Overall budget plan
• Project narrative with needs assessment, work plan, impact analysis, evaluation, sustainability plan
• At least three SMART goals with two objectives each in work plan
• Letters of commitment for non-rural/out-of-state applicants
• Compliance with State of Nevada vendor registration
• No indirect or construction costs; administrative expenses capped at 10%; no pre-award costs
• Applicants must be registered Nevada vendors.
• Must demonstrate impact on provider training and retention in rural areas.
• year rural service commitment for all program recipients.
• Vendor registration with State of Nevada.
• Submission through RFA process.
