Project ECHO for Maternal and Behavioral Health (Rural Health Transformation Program)

Summary

The Minnesota Department of Health Office of Rural Health and Primary Care is soliciting applications for Project ECHO programming focused on maternal and behavioral health for rural clinicians and medically underserved communities across Minnesota. This initiative, funded 100% by CMS/HHS, seeks to expand rural provider education and capacity through virtual, case-based sessions (12–24 sessions per grantee, monthly), with an emphasis on health equity, network-building, and workforce development. Partnerships with Tribal Nations and organizations serving underserved rural communities are encouraged.

Eligibility

Eligible applicants include public or private entities with capacity to deliver ECHO programming, including but not limited to: hospitals, health care provider organizations, Federally Qualified Health Centers (FQHCs), Tribal health organizations, professional associations, academic medical centers, and nonprofit organizations with demonstrated experience serving rural or medically underserved populations in Minnesota.

Requirements Checklist

• Applicants must propose a 12–24 session, monthly Project ECHO curriculum focused on maternal health (including maternal mental health) and mental/behavioral health. Activities must target rural clinicians and emphasize reach to underserved communities. The application requires: (1) a workplan and detailed budget for Year 1 (budget period 1); (2) summary of a high-level 5-year approach; (3) description of partnerships/collaborators; (4) approach to health equity and workforce development; (5) letters of support or documentation of relevant experience; (6) statement on avoidance of duplicative programming. Applicants proposing partnerships with Tribal Nations must include demonstration of collaboration.
